To:

wp_enqueue_script(uniqid(),$jsurl.$file,array(‘jquery’));

I also updated line 40.
before only the x are not showing
now both x and checkmarks are no longer showing =(

I have them showing up, but they are NOT transparent. They have a white background for some reason. Please advise.

If not transparent, the back ground is coming from your theme. But i see you already adjusted it.

I have changed line 40 to wp_enqueue_script(uniqid(),$jsurl.$file,array(‘jquery’)); as WP Eden said bit dont work.
Also i disabled all plugins as some may interfier and no luck at all. Same problem here
